Description of Benefits
Dental:
Provides 10 to 50 percent off on virtually all dental services including
specialty care. The network is comprised of approximately 28,000 credentialed
dentists providing nationwide coverage.
Vision:
Members receive up to a 50 percent discount on eyeglasses, contact lenses
(excluding disposable) and other retail eyewear items through a network
comprised of over 9,000 eye care professionals nationwide. In addition,
members can receive discounts on eye examinations and surgical procedures
including Lasik where available. Replacement contact lenses is available
through a convenient mail order service.
Hearing:
Provides discounts on exams, up to 20 percent on hearing aids and related
supplies along with a free annual cleaning and check of hearing aids purchased
through the program. The network is comprised of approximately 3,000 licensed
dispensing audiologists.
Prescription Drugs:
Provides discounts of up to 30 percent on both brand and generic drug prices
through a network of over 40,000 participating provider locations.
Chiropractic:
Provides discounts on exams, x-rays and up to 40 percent off diagnostic
services. Save up to 20 percent on all other services. It is one of the
largest network of credentialed chiropractors providing nationwide coverage.
Vitamins and Nutritional Supplements:
Provides discounts on a variety of vitamins, nutritional supplements and
non-prescription medications through our mail order program at an average
savings of up to 50 percent below suggested retail prices.
Medical Information Telephone Service:
24-hour toll-free access for non-directive medical information. The 24-hour
nurse helpline provides access to Registered Nurses who currently average
more than 15 years experience. The nurse helpline is available 24 hours
a day, 7 days a week. These nurses are specially trained to offer prompt,
confidential medical counseling to assist members in making informed decisions
on their healthcare needs.
Home Medical Equipment:
Provides up to a 20 percent discount on a variety of products including:
ambulatory aids, respiratory therapy products and services, diabetic supplies,
orthopedic products, long term rehabilitation equipment, supplies and aids
to daily living and many more items.
Medical Records Storage and Retrieval Service:
Provides a "safe deposit box" for vital health information and documents
including medical conditions, allergies, medications, emergency contacts,
physician contacts, insurance information and advanced directories such
as living will and organ donation authorizations. Information can be updated
at anytime. Information will be released to the member or qualified healthcare
provider via a 24-hour toll-free telephone number.
Physician Referral Program:
Provides referrals to qualified credentialed physicians through a national
network of general practitioners and specialists with average savings of
20-25%, up to 40%.
Alternative Medicine Referral Program:
Provides referrals to qualified alternative medicine practitioners including:
acupuncturists, herbologists, massage therapists, nutritionists and more.
Extended Care Program:
The extended care program provides access to quality extended care and home
healthcare providers with savings on nursing facilities, assisted living
facilities, home healthcare, respite care, homemaker services and much more.
Travel:
Over 3,100 participating hotels and motels across the United States offer
savings of up to 50 percent. The travel benefit also offers discounts of
up to 30 percent on car rentals through several national rental agencies.
