Refer a Friend:
For each friend you refer to the program who becomes a member, you will receive a $10 cash reward.
How to Claim your Reward. It's fast and easy. Just follow these 3 simple steps.
1. Purchase your Family Healthcare Discount Plan
2. Enter your name, e-mail address and membership number (this is your SSN)
3. Enter name, e-mail address of your friend or associate and the name of their group (company, association, nonprofit organization, union, etc.)
Refer a Group:
If you refer a company, association or organization of any type and their employees, associates or members purchase the Family Healthcare Plan, you will be rewarded in proportion to the number of people in the company, association or organization who become members:
If your friend and/or friend's group becomes a member, ask him/her or the group to fill in your name and e-mail address on their enrollment form. (Rewards cannot be paid without this information.)
That's all that needs to be done to claim your reward. We will automatically track any member or organization that provides us with the information and we will happily pay a reward to the first person who provides us the referral. And remember, there is no limit on how many referrals you make and how many rewards you can claim.
So hurry, you'd hate to lose all that cash if someone else referred a large group that could have been your referral. You should start by making a list of all the groups you belong to: church and religious groups, your company, your union, YMCA, charitable groups, school groups, Boy Scouts and Girl Scouts, etc.
Number of People Who Become Members Cash Reward
25 - 100 people = $250
101 - 500 people = $1,000
501 - 1000 people = $5,000
1001 or greater = $10,000
